One of the crucial issues facing social security institutions are administrative costs, which constitute a major category of expenditure in total health insurance spending. Public budgets are under pressure around the world as resources become scarce, particularly due to the impact of the pandemic. Effectively controlling health expenditure with limited resources is essential to ensure the sustainability of health insurance systems. According to OECD Health Statistics (2014), the average administrative cost of the social health insurance system in OECD countries is 3 per cent, and the shares are much higher for low- and middle-income countries.

Administrative costs include staff costs, buildings and equipment, information technology (IT), operational (maintenance and utilities) and programme costs (research, communications). Reducing administrative costs by simply cutting human resources cannot be an alternative and the use of innovative measures is key. In addition, improving administrative cost efficiency is a way of using resources to expand health care services.

By examining various variables that can affect administrative costs and looking into country cases, we can identify various ways to increase health care service efficiency.
